# AI project evidence template

Use this to document a workplace task or an independent demonstration. Do not include personal, confidential or employer-owned information you lack permission to share. A fictional recreation is practice work, not proof of a result in your job.

## Project

- Project name:
- Project type (employment, independent practice, volunteer or student):
- Intended use (internal review, job application or public portfolio):
- What information may be shared with this audience?
- Date and tool/version, if known:
- Relevant job or task:

## Problem and input

- Who needs this output and why?
- What material did you start with?
- Is it fictional, public or otherwise permitted to share?
- What would a useful result look like?

## Process

- What did you do yourself?
- Which step did AI assist with?
- First prompt:
- Revised prompt and reason for the change:

## Evidence

- Sample input:
- Initial output:
- Your edits:
- Final output:
- Links or files a reader can safely inspect:

## Checks and limitations

- Which facts or calculations did you verify?
- What test would expose a bad answer?
- What failed or remained uncertain?
- Which decision still requires a person?
- What would you improve next?

## Results

Describe the deliverable. Include numerical benefits only if you measured them, with your method and its limits. A practice result is not evidence of performance in a live organisation.

## CV bullet

[Action you took] + [specific task] + [how AI helped] + [your check or documented result].

Can you explain every claim and show the evidence in an interview?
